Since Corbeil-Essonnes, the Head of State has notably affirmed his desire to get out of activity-based pricing as of the next budget text.

The Elysée had promised “very concrete” measures. This Friday, Emmanuel Macron sent his wishes to caregivers from a hospital in Corbeil-Essonnes (Essonne), while the health sector is going through an unprecedented crisis in France.

Between the strike of liberal doctors and the crisis that emergency hospital services are going through everywhere on the territory, the head of state was expected at the turn. In the introduction to his speech, he showed his desire to “get out of this day of endless crisis”.

• Recruit 6000 medical assistants

First announcement from the Head of State: the recruitment of thousands of new medical assistants. The latter, created in 2018, should allow doctors to free up medical time to focus on care, recalls the health insurance website.

Emmanuel Macron therefore promised to “accelerate” their recruitment. While there are nearly 4,000 today, they are expected to increase to 10,000 throughout the territory “by the end of next year”, said Emmanuel Macron.

“It’s a real success” of the “My Health 2022” plan presented at the start of the previous five-year term, “it frees up medical time” for caregivers to follow their patients, said the head of state.

• Reorganize work in the hospital

“By June 1”, Emmanuel Macron wants a reorganization of work in the hospital, in order to make jobs in the health sector more attractive but also to avoid resignations.

“We must do everything to keep caregivers,” said Emmanuel Macron. Among the projects mentioned: “hyper-rigidity” in the application of 35 hours, leading to a system which “only works with overtime”, according to him.

• A doctor/administrative duo at the head of hospitals

It is a “project” unveiled by Emmanuel Macron this Friday. That of placing an “administrative and medical” tandem at the head of hospitals.

“For a very long time we said ‘it should not be the caregivers who run the hospital, it must be the administrators’. Then we said the problem is the administrators who run”, he observed during his wishes to caregivers.

“I wish that we could (…) put an administrative and medical tandem at the head of our hospitals, a real tandem on the basis of a project”, he insisted.

• Get out of activity-based pricing

Criticized by many caregivers, activity-based pricing in care services is set to disappear, as Emmanuel Macron said this Friday during his wishes to caregivers.

He announced the “exit” of this system from the next Social Security budget, in favor of funding “on public health objectives.

“For the public hospital, there must be a structuring part of the remuneration which is based on public health objectives which are negotiated at the scale of a territory”, declared the head of state. .

• Better remunerate doctors who provide on-call

Although presenting his wishes from a hospital, Emmanuel Macron wished to support the role of general practitioners, some of whom have been on strike since the end of December to demand a doubling of the price of the consultation.

The head of state said he wanted “better pay” for city doctors who provide ongoing care and “take care of new patients”, so that the French “can easily find a doctor on call”.

“We are going to pay better the doctors who ensure the permanence of the care and those who take care of new patients”, explained the president.

• A general practitioner for all French people with a chronic illness

All patients suffering from a chronic illness and who do not currently have a treating doctor will be offered one “before the end of the year”, Emmanuel Macron finally announced.

“We have 600,000 patients with chronic diseases who have no attending physicians and that’s a real problem,” lamented the head of state.

These patients “will be offered a treating doctor before the end of the year” or failing that “a treating team”, he added.
